Thank you for choosing Mad Macs Inc for your computer or electronic device repair. Our goal is to have all clients preregister to minimize the time spent dropping off and picking up in our showroom.

Please Note: A minimum Diagnostic Charge apply for all out of warranty product repairs!

Should you choose to repair your device with us, a diagnostic charge will be billed as part of your repair, not in addition to. If you choose not to have your item repaired, this is the cost you will be charged for our services! We do not diagnose for free!

$65 for all out of warranty computer diagnostics i.e. Laptops/ Desktops.
$15 for all out of warranty iPhones, iPads, Tablets and IOS Products.

Mad Macs Inc. accepts no responsibility for data of any kind. We strongly recommend you back up your computer or device on a regular basis. Has your data been backed up before bringing your unit in for service? Please select one of the following:

Physical Damage to Unit

Please list any noticeable damage to your device including dents, large scratches, cracks, breaks, etc.. Note: Your unit will be reinspected when dropped off. If damage is observed and not listed you will be notified before repair begins.

Liquid Damage

Has your device been exposed, spilled on, dropped in, and/or in any way in contact with liquid of any type or nature? Is this liquid toxic in any way or harmful to humans?

Apple Security Turned Off -(Mandatory Do not skip as charges will apply! )

If bringing an Apple product (Phones/ iPads/ Macs) in for services of any kind, we require you to TURN OFF the "Find my device" security feature located in iCloud settings or in "Find my" in Settings! THIS IS MANDATORY! Failing to do so, requires additional time and slows the repair process down for all. If you have forgotten your password and need us to reset it, this fee will apply as well. $25 per device.

COVID 19 CLEANING CHARGE

Mad Macs, do to the exorbitant cost of supplies, are charging a small $1 to $2 dollar cleaning Fee for all devices. Laptops, iPads, IOS Devices $1, Desktops $2.

Blanket Acceptance Statement

By agreeing to the following, your giving permission to Mad Macs inc. And its employees and contractors to perform any necessary repairs, updates, diagnostics, etc. to your devices as required. This statement is provided in place of our standard sign-in forms. By submitting this form, we are accepting it as your signature, agreement, and authorization for this transaction. If you choose not to sign this statement, we will not proceed with your repair. If you have any questions, please ask at the time of drop off.
